Job Title: Môn Community Link Support Officer
Hours:37 hours per week
Salary: SCP Point 19 - £31,067
Pension: 17% of annual salary to Medrwn Môn’s approved pension scheme
Annual Leave: 25 days per annum annual leave, plus bank holidays, plus 4 additional days
Place of Work: Office based, Anglesey Business Centre , Llangefni, LL77 7XA

Job purpose:
Môn Community Link is a service that Medrwn Môn provides to individuals and professionals, providing information to them on third sector organisations and the health, social care and wellbeing related services they offer. Môn Community Link is part of the Single Point of Access project, in partnership with the Isle of Anglesey County Council and Betsi Cadwaladr University Health Board.
On a day-to-day basis you will deliver the Môn Community Link helpline service to users, ensuring service delivery is of the highest standard and in line with Medrwn Môn’s ethos, values and aims.
Main Responsibilities and Tasks:
• Responsible for answering calls to the Môn Community Link helpline
• Respond appropriately to telephone, written and email enquiries
• Maintain the Elemental Social Prescribing Software managed list of referrals
• Making clear and comprehensive notes of conversations so that information about the service user’s needs are clear
• Refer service users to relevant third sector organisations that can support them with their needs
• Ensure the follow-up process and outcomes monitoring system is up-to-date
• Relevant fact-finding and relationship-building in order to establish the service user’s needs and identify relevant opportunities/services
• Maintaining and building the service user database by confirming information about the contact and their details
• Ensure that a pro-active casework approach is adopted by supporting service users through necessary processes and identify and confirm the outcomes of their enquiry both during the conversation and in the follow-up process
• Provide information on third sector organisations to health and social care professionals
• In conjunction with the Link Team, monitor the service user’s journey through developing and maintaining feedback systems, sharing insight with other staff/partners as appropriate
• Monitor and evaluate service provision and statistics using Key Performance Indicators (KPI’s) on a monthly, quarterly and annual basis
• Present quantitative and qualitative statistical analysis of Môn Community Link’s data and information. Collate, analyse and report on service statistics on a regular basis to inform strategic and operational planning and monitoring
• Relevant fact-finding and relationship-building with third sector organisations, organising meetings and encouraging them to complete Môn Community Link’s Registration Form
• General office duties and administration
Knowledge and understanding:
It is essential that candidates can demonstrate the following knowledge and understanding:
• The local voluntary sector and the work that voluntary organisations, community groups and volunteer-involving organisations undertake - in particular those that are involved in the health, social care and well-being sector
• The relationship between the voluntary sector and the statutory sector
• Working with people with a range of characteristics/situations that make adults vulnerable, such as mental health, learning disabilities, ill-health and medical conditions, stressful circumstances such as bereavement etc.
• The Data Protection Act and a practical commitment to it
Person Specification
A well-organised individual who will take responsibility for key parts of the organisation’s internal administrative procedures. The postholder should have demonstrable experience of working in a busy office environment, excellent attention to detail and experience of managing a demanding workload with competing deadlines.
An effective communicator who can make decisions and recommendations regarding the administrative functions of Medrwn Môn. An excellent standard of written and verbal communication is essential alongside the ability to maintain an enthusiastic, ‘can-do’ attitude.
